Falls Diary for High Risk Residents

Sometimes the most simple tools are the best. We were looking for a quick and easy way of recording whether or not our high-falls risk residents had fallen on particular days to see at a glance if we could determine a pattern (e.g with regards to staff, activities, etc).

We put together this clear A4 document which can be used for four weeks. It includes space each day to record whether or not the resident has fallen (circle Y/N), the details of the fall and a signature, alongside space for additional details at the bottom (e.g. referred to OT on X date). We’ve found it a really useful tool which slips in neatly alongside the daily record within a resident’s notes, hopefully you will find it handy too!
